Iowa’s CHCs Awarded Quality Recognition Badges
Iowa’s community health centers (CHCs) continue to be leaders in helping patients achieve their healthiest lives. Each year, the federal Health Resources and Services Administration (HRSA) recognizes outstanding achievements by Health Center Program awardees in the areas of quality, access, health outcomes, and health information technology for the most recently completed calendar year by awarding Community Health Quality Recognition (CHQR) badges.
This year:
100% of Iowa’s 14 health centers received at least one recognition.
Three (3) of Iowa’s 14 health centers achieved the Gold Health Center Quality Leader badge, meaning they perform in the top 10% of all community health centers nationwide.
Three (3) achieved the Silver Health Center Quality Leader badge, performing in the top 11-20% nationally.
“Iowa’s community health centers continue to set the standard for excellence in community-based care nationwide through their commitment to providing high-quality, affordable health care across the state,” said Aaron Todd, Chief Executive Officer of the Iowa Primary Care Association. “This recognition from HRSA reflects the dedication of the providers, staff, and care teams who work every day to ensure Iowans have access to the care they need. We’re especially proud that all 14 of Iowa’s community health centers received at least one award this year, an achievement that highlights the strength and impact of Iowa’s health center network.”
Iowa’s community health centers received the following recognition:
National Quality Leader (6 awardees): awarded to those achieving the best overall clinical quality measure (CQM) performance among all health centers. Health centers in the top 30% are recognized in the following tiers: Gold (top 10%), Silver (top 11-20%), and Bronze (top 21-30%).

Improving Health Care Access (4 awardees): awarded to those who are meeting at least one of these criteria – earn at least one Health Center Quality Leader or National Quality Leader badge and improve by at least 15% in back-to-back Uniform Data System (UDS) reporting years or increase by at least 5% in back-to-back UDS reporting years for total patients and patients receiving mental health, substance use disorder, vision, dental, or enabling services.

Advancing Health Information Technology for Quality (13 awardees): awarded to those who offer telehealth services, exchange clinical information online with key providers’ healthcare settings, engage patients through Health Information Technology, and collect data on patient health related needs.

National Quality Leader – Behavioral Health (3 awardees): awarded to those meeting or exceeding two of the three CQM performance targets for behavioral health, including depression remission at twelve months, screening for depression and follow-up plan, and Screening, Brief Intervention, and Referral Treatment (SBIRT).

People’s Community Health Clinic (Waterloo, Clarksville, and Traer areas)
National Quality Leader – Diabetes (2 awardees): awarded to those meeting or exceeding two of the three measure targets - Adult Body Mass Index (BMI) screening and follow-up plan, weight assessment and counseling for nutrition and physical activity for children and adolescents, and diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9.0%).

Proteus, Inc. (Des Moines, Fort Dodge, and Iowa City areas)
National Quality Leader – Heart Health (6 awardees): awarded to those meeting or exceeding three of the four Million Hearts Goals measure targets for tobacco use screening and cessation intervention, ischemic vascular disease (IVD) aspirin or antiplatelet use, statin therapy for prevention and treatment of cardiovascular disease, and blood pressure control.

Preventive Health Badge (2 awardees): awarded to those meeting or exceeding five of these ten measure targets – dental sealants for children between 6-9 years, weight assessment and counseling for nutrition and physical activity for children and adolescents, childhood lead test screening ages 9-72 months, childhood development screenings and evaluations ages 0-5 years old, screening for depression and follow-up plan, tobacco use: screening and cessation intervention, Adult Body Mass Index (BMI) screening and follow-up plan, breast cancer screening, cervical cancer screening, and colorectal cancer screening.
