Iowa’s Community Health Centers: National Quality Leaders

Iowa’s community health centers continue to be leaders in helping patients achieve their healthiest lives. Each year, the federal Health Resources and Services Administration (HRSA) recognizes outstanding achievements by Health Center Program awardees in the areas of quality, access, health outcomes, and health information technology for the most recently completed calendar year by awarding Community Health Quality Recognition (CHQR) badges.

This year:

  • Three (3) of Iowa’s 14 health centers were recognized with the Gold Health Center Quality Leader badge, meaning they perform in the top 10% of all community health centers nationwide.

  • Two (2) achieved the Silver Health Center Quality Leader badge, performing in the top 11-20% nationally

Altogether, 7 of the 14 Iowa’s community health centers – 50% of our members – achieved recognition in the top 20% of the over 1,400 community health centers nationwide. This is an amazing accomplishment!

Iowa’s community health centers received the following recognition:

  • National Quality Leader (5 awardees): awarded to those achieving the best overall clinical quality measure (CQM) performance among all health centers. Health centers in the top 30% are recognized in the following tiers: Gold (top 10%), Silver (top 11-20%), and Bronze (top 21-30%).

    • Gold:

      • Community Health Care (Quad Cities, Clinton, Muscatine areas)

      • Peoples Community Health Clinic (Waterloo area)

      • Promise Community Health Center (Sioux Center area)

    • Silver:

      • Community Health Centers of Southeastern Iowa (Burlington area)

      • Siouxland Community Health Center (Sioux City area)

  • Health Center Quality Leader – Behavioral Health (1 awardee): awarded to those meeting or exceeding two of the three CQM performance targets for behavioral health, including depression remission at twelve months, screening for depression and follow-up plan, and Screening, Brief Intervention, and Referral Treatment (SBIRT).

    • Community Health Care (Quad Cities, Clinton, and Muscatine areas)

  • Health Center Quality Leader – Heart Health (4 awardees): awarded to those meeting or exceeding three of the five CQM performance targets for heart health, including tobacco use screening and cessation intervention, Ischemic Vascular Disease (IVD), statin therapy for the prevention and treatment of cardiovascular disease, and controlling high blood pressure (hypertension control).

    • Community Health Care (Quad Cities, Clinton, and Muscatine areas)

    • Community Health Centers of Southeastern Iowa (Burlington area)

    • Promise Community Health Center (Sioux Center area)

    • Siouxland Community Health Center (Sioux City area)

  • Improving Health Care Access (3 awardees): awarded to those who meet at least one of these criteria – earn at least one Health Center Quality Leader or National Quality Leader badge and improve by at least 15% in back-to-back Uniform Data System (UDS) reporting years or increase by at least 5% in back-to-back UDS reporting years for total patients and patients receiving mental health, substance use disorder, vision, dental, or enabling services.

    • Community Health Care (Quad Cities, Clinton, and Muscatine areas)

    • Community Health Centers of Southeastern Iowa (Burlington area)

    • Eastern Iowa Health Center (Cedar Rapids area)

  •  Advancing Health Information Technology for Quality (7 awardees): awarded to those who offer telehealth services, exchange clinical information online with key providers’ healthcare settings, engage patients through Health Information Technology, and collect data on patient health related needs.

    • Community Health Care (Quad Cities, Clinton, and Muscatine areas)

    • Community Health Centers of Southeastern Iowa (Burlington area)

    • Eastern Iowa Health Center (Cedar Rapids area)

    • Peoples Community Health Clinic (Waterloo area)

    • Primary Health Care (Des Moines, Marshalltown, and Ames areas)
